Does anyone operate the phone with the FritzBox 7490? Just register it in the router or does it need anything else? I understand that I don't need a base station as that should be the FB. Right?

Yes, the Fritzbox is the base and you register the gigaset handset as a Dect phone on it. I also operated it like this for a long time.

As you describe it, I operate Gigaset S850HX and FritzFon together with a FritzBox 7590.
