Question about Synology Ds118

Is it possible to operate this unit without an inserted hard disk, but with an external SSD connected via USB? (In case anyone finds this question strange: I have just spent over 300 francs on an external SSD to operate as a NAS on the Swisscom Internet Box and have now learned that Swisscom is discontinuing this function from March).

Could work, but I wouldn't assume so.
These things are simply designed to have an internal hard drive.

Did you buy a normal 3.5" external hard drive? Manufacturer/model?
If you have a normal hard drive, you could remove it from the enclosure and simply install it in the Synology.

No, it is not possible because the operating system is on the hard disk.

But instead of 3.5" hard disks, 2.5" hard disks can also be installed. If even mechanical 2.5" drives are too loud, you can also use 2.5" SSDs.
